Reporting your Data is Vital
To track naloxone or other FDA-approved opioid antagonist distribution efforts, we ask that all participating pharmacies report kit distribution by quarter, even if the number is 0. Thanks for your cooperation so we can see and measure the impact we’re making on overdose deaths.
A representative from your pharmacy must fill out the simple form below during these four reporting periods:
June 1 – August 31
Due September 30
September 1 – November 30
Due December 31
December 1 – February 28
Due March 31
March 1 – May 31
Due June 30
